If you’ve come this far, you have no doubt given a lot of thought to nominating a teacher who has either made a significant difference in your life or the life of someone close to you.  Congratulations.

Nominations need to be submitted by Friday, February 27, 2009 on the 2009 Nomination Form, preferably typed.  If you must hand write your nomination, the writing must be legible.  Nominations that cannot be read will not be considered.

The nomination form should be either downloaded onto your computer and filled out (the form should guide you) or printed out if hand writing your nomination.  Once completed, nominations can be e-mailed to robin@omahafoundation.org (please type Excellence in Teaching Award Nomination in the subject line) or mailed to:  Omaha Community Foundation, Attn: Robin Spitznagle, 302 S 36^th Street, Suite 100, Omaha, NE  68131.  Nominations must be received no later than Friday, February 27, 2009, at 5:00 p.m.
